Notice Of Intended Marriage

Filling this form out is the first step in the process of getting married. Your celebrant should be able to help you fill out this form.  I have the capability of sending you a link and having you do it all online.

You are more then welcome to fill this form out yourself BUT DO NOT SIGN IT.  It must be witnessed by specific people, eg celebrant, Police Officer, qualified Doctor, qualified Lawyer etc.

Once this has been filled out your Celebrant will need  to see evidence of your date of birth and your identity.  This can be done via a birth certificate and drivers licence or your passport.  If you have previously been married your Celebrant will also need to see proof that this marriage has ended.

Happily Ever Before and After

As soon as practicable after receiving the Notice of Intended Marriage, your celebrant must give you a document outlining the obligations and consequences of marriage. This document has been approved by the Attorney‑General in the form of a brochure entitled Happily Ever Before and After, and indicates the availability of marriage education and counselling and other important legal matters concerning marriage.

Marriage Application Form - for Western Australia only

If you decide to change your name once you get marriage you will need to get your hands on a copy of your marriage certificate.  The one you get given on the day of your wedding is purely for presentation purposes only and has no legal weight to prove that you are in fact married.

You will need to fill out the below form, provide specific forms of certified identification and send it off to Births, Deaths and Marriages along with the sum of $48.  They will then send you your certificate back and you can go about change your name.  There are some forms here to help you with that process.

Joint Application to Add Marriage Details in a Child's Birth Registration - for Western Australia only

If you and your husband had children before you married, your marriage details will not be listed on their birth certificates.

To add these details to their birth certificates you will need to fill out a Joint Application to Add Marriage Details in a Child's Birth Registration.

As with the Marriage Application form specific forms of certified identification are required and it costs a fee to get these details added to your child's birth certificate. Something important to remember is that if your child is over 12 years of age they will to sign this form giving their consent for these details to be added.
